CHP 190 Application for Release of Information
If your accident was investigated by the California Highway Patrol, use form CHP 190, Application for Release of Information, to request a copy of your official Traffic Collision Report. This form allows the CHP to release the report to you for insurance, legal, or personal records purposes.
REG 488C - Application for Salvage Certificate or Non-repairable Vehicle Certificate
When a car is deemed a total loss, use this form to tell the DMV that your car was totaled but you decided to keep it. The form marks the vehicle as either salvage (can be repaired and registered again) or nonrepairable (only for parts and must be scrapped).
