California DMV SR-1 Accident Report Form

If you were involved in a traffic accident in California that caused an injury, death, or more than $1000 in property damage, you are legally required to report the crash to the DMV within 10 days. Missing this deadline can put your driving privileges at risk.

California DMV SR-19 Financial Responsibility Information Request Form

When insurance coverage is in question after a California traffic accident, the DMV SR-19 Financial Responsibility Information Request Form allows you to request official confirmation of whether the other driver was insured at the time of the crash. This can be especially important in uninsured motorist and hit-and-run situations.

CHP 190 Application for Release of Information

If your accident was investigated by the California Highway Patrol, use form CHP 190, Application for Release of Information, to request a copy of your official Traffic Collision Report. This form allows the CHP to release the report to you for insurance, legal, or personal records purposes.

REG 488C - Application for Salvage Certificate or Non-repairable Vehicle Certificate

When a car is deemed a total loss, use this form to tell the DMV that your car was totaled but you decided to keep it. The form marks the vehicle as either salvage (can be repaired and registered again) or nonrepairable (only for parts and must be scrapped). You must file this form within 10 days of the total loss settlement.
